This notice along with a listing of the maximum premiums per $1,000 coverage plus policy fee will be part of your final application, as supplied by Starmount. Your signature will be requested at that time.

Premiums may change.

I understand that the current premium is guaranteed for the first year. The current premium may increase in years 2-10 but not above the guaranteed maximum premium. The rates will change in policy years 6 and 11 as I enter a new age band. All premiums are guaranteed for the life of the policy beginning in year 11. I understand that any premium change will be by class, and no individual will be singled out for a change.

In most states, insurance will not be in force until the application has been approved by the company and the policy issued and delivered to the insured while they are in the same health condition as described on their application, and the first premium paid. However, in Kansas, the insurance in force will be limited to $1,000 and will be in force upon receipt of an application and a premium by the company.

The Accelerated Benefit offered under this coverage may and may not qualify for favorable tax treatment under the Internal Revenue Code of 1986 or as amended thereafter. Whether such Accelerated Benefit qualifies depends on factors such as Your life expectancy at the time death benefits are accelerated. Also, it depends on whether you use the Accelerated Benefit to pay for necessary long-term care expenses, such as nursing home care. If the Accelerated Benefit qualifies for favorable tax treatment, the benefits will be excludable from your income and not subject to federal taxation.

Tax laws relating to Accelerated Benefits are complex. You should consult with a qualified tax advisor about circumstances under which You could receive an Accelerated Benefit excludable from income under federal law. Receipt of an Accelerated Benefit may affect your or your family's eligibility for public assistance programs. These may include medical assistance (Medicaid), Aid to Families with Dependent Children (AFDC), supplementary social security income (SSI), and drug assistance programs. You should consult with a qualified tax advisor and with social service agencies concerning how receipt of such a payment will affect your or your family's eligibility for public assistance.

Premiums cannot change because of my age or health. I understand this policy�s premium rates may change. The maximum premium that may be charged is shown in the policy and it is higher than the premiums currently scheduled to be charged. The premium will not be changed more than once in a policy year and notice of any premium change will be provided before the policy year in which the change is made. No individual will be singled out for change. Changes occur only if an entire class changes.

* Lasts until age 95. Your cost will never go up because of your age or health. Only if everyone in your classification has a rate change can your cost change. You cannot be singled out.
